Distinction of acanthocytes from echinocytes based on 3D confocal images and classification with artificial neural networks. The 3D confocal imaging reveals the characteristic irregular spicule morphology of acanthocytes in a patient with Huntington's disease neuroacanthocytosis. (human (peripheral blood, Huntington's disease neuroacanthocytosis); Figure 1.)

Peripheral blood smear showing acanthocytes in patients with PKAN (pantothenate kinase-associated neurodegeneration) and McLeod syndrome. Representative light micrographs comparing red blood cell morphology: acanthocytes (spiculated cells) from neuroacanthocytosis patients vs normal discocytes from controls. (human (peripheral blood, PKAN/McLeod neuroacanthocytosis); Figure 1, B.)

Three-dimensional confocal microscopy images distinguishing acanthocytes from echinocytes, with artificial neural network classification. Acanthocytes (left panels) are erythrocytes with irregularly spaced, spicule-like projections of varying length that do not retract with dilution, as seen in neuroacanthocytosis and hypobetalipoproteinaemia. Echinocytes (right panels) have regularly spaced, shorter crenations that retract under physiological conditions. Confocal microscopy enables 3D resolution of surface projections. (human; Figure 1.)
